At a roundtable discussion on the future of surrogacy operations, Patriot Conceptions hosted a conversation about AI, automation, and real-time matching. That is where I want this story to begin: not with a speculative technology demo, but with people in the field asking how this industry should actually work.

AI is only useful here if it can survive contact with the room: clinicians, coordinators, intended parents, surrogates, donors, attorneys, escrow partners, and operators who live with the consequences of every unclear handoff.

There is another image that belongs near the beginning: a pen.

When OpenAI sent the ChatGPT Pro "Since '24" pen to its earliest subscribers, it was a small physical signal from a company building at the frontier. For Patriot Conceptions, it also became a useful reminder. We have not treated AI as a trend to watch from a safe distance. We have tried to stand near the frontier, use the tools early, test them under real operating pressure, and ask a harder question than whether the technology is impressive.

The question is: can it help us keep our promises?

In fertility and surrogacy, the promise is not abstract. Intended parents need clarity when the path is uncertain. Surrogates deserve respect, structure, and timely communication. Donors, clinics, attorneys, coordinators, escrow partners, and nurses all need context that does not disappear between handoffs. Families need people who can move fast without becoming careless.

That is the reason AI matters to us. Not because it looks futuristic. Because the work is too human to leave buried inside fragmented inboxes, PDF attachments, isolated spreadsheets, missed calls, and memory.

There is a difference between using AI and becoming AI-native.

Using AI means asking a model to draft a sentence, summarize a document, generate an image, or answer a question. Those are useful acts. They save time. They expand the surface area of a small team.

Becoming AI-native is different. It means redesigning the way the company thinks, routes, records, escalates, checks, and learns. It means the technology is not a sidecar. It becomes part of the operating discipline.

That shift has been the real work at Patriot Conceptions.

We use AI to think faster, but the goal is not speed alone. We use it to make context durable. We use it to reduce ambiguity before it becomes anxiety. We use it to help the team see the next decision, the current risk, and the missing piece of information. We use it to make operations more legible, so a family-building journey does not depend on one heroic coordinator remembering every thread by hand.

Healthcare-adjacent work cannot be built on vibes. It needs privacy, judgment, boundaries, auditability, and human ownership. AI that cannot respect those constraints is not ready for this category.

So our approach is simple: frontier tools, disciplined use, human accountability.

Our IVF stimulation and medication-tracking direction looks like a mobile interface, but what it really represents is a principle: the next decision should be clear.

Family-building care often gives people too many fragments. A portal message here. A medication label there. A PDF instruction sheet. A phone call. A clinic note. A calendar invite. A nurse's clarification. A changed dosage. A monitoring visit. A trigger shot window. A retrieval estimate. Each piece may be correct, but the lived experience can still feel chaotic.

The design direction we are pushing toward separates what is clinic-confirmed from what is estimated. It shows the next dose before the long list. It makes status explicit. It gives the user a contextual support path, not a generic help button floating somewhere else.

That is AI work even before a model appears on screen.

Good AI products are not just smarter chat boxes. They are systems that decide what should be visible, what should be hidden, what should be confirmed, what should be escalated, and what should never be guessed.

The same standard applies to case matching.

That is one of the most important areas where the surrogacy and donor world needs a higher standard. Matching is too often presented as a browse-and-select experience, as if the most important work is choosing from a catalog. But the actual decision is much more serious.

An intended parent is not only asking, "Do I like this profile?"

They are asking whether the journey can be safe, aligned, legally sound, clinically workable, emotionally sustainable, financially clear, and properly supported by a team that sees the whole case.

That requires richer context: medical clearance, psychological readiness, insurance, availability, attorney intake, clinic timing, state-law considerations, communication preferences, open questions, non-negotiables, and changes since last review.

This is where AI can be transformative if it is used with discipline. It can surface signals. It can flag missing information. It can help a coordinator prepare a better explanation. It can compare cases against explicit criteria. It can show why a match is ready, why it is not ready, or why it needs human review.

But AI should not pretend to be the moral decision-maker.

The goal is not to remove judgment. The goal is to give judgment better context.

The most visible AI demos usually happen at the front end. A chatbot. A shiny assistant. A generated image. A beautiful product surface.

But in our business, trust is often won in the back office.

Did the fax get received? Was it assigned to the right person? Did someone know it was urgent? Was the record attached to the right case? Did the team see the new document before the next decision? Did the family get an answer before anxiety filled the silence?

That is why the PC Ops Notifier image is deliberately privacy-safe. The point is not operational detail. The point is the operating pattern.

AI should help route work before it becomes a crisis. It should help classify inbound documents, alert the right team member, and keep the case moving without forcing people to babysit every queue. It should make the invisible work visible enough to manage.

The industry often talks about AI as if it will replace care. I think that is the wrong frame.

At Patriot Conceptions, the better use of AI is to protect care from operational drag.

The AI-generated image in this issue is aspirational on purpose.

It imagines a future where fertility and surrogacy operations are not scattered across disconnected tools, but coordinated through a privacy-first command layer. Intake, clinic records, medication timing, legal milestones, case matching, fax triage, escrow, insurance, and care-team communication should not feel like separate worlds to the people living the journey.

What should shock the industry is not a more theatrical image of technology.

What should shock the industry is a higher operating expectation.

Every family should know what is happening next. Every surrogate should know who is responsible. Every coordinator should have the context needed to make a better decision. Every clinic update should land where it belongs. Every estimate should be labeled as an estimate. Every confirmed instruction should be clearly distinguished from a forecast. Every sensitive record should be treated as sensitive. Every AI-generated output should have a human owner.

If that sounds ambitious, it should. This field deserves ambition.

The roundtable photo matters because it shows the other half of the work.

We are not building this in isolation. We are bringing the conversation into the rooms where surrogacy professionals, clinic operators, lawyers, coordinators, and intended-parent advocates are already asking what the next operating model should be.

The sign on the table says the conversation was powered by Patriot Conceptions. The topic was the future of surrogacy operations: AI, automation, and real-time matching.

That is exactly where I want us to keep pressing.

Not AI as theater. AI as better care infrastructure.

Not automation as cold efficiency. Automation as fewer dropped handoffs, fewer status mysteries, fewer preventable delays, and more time for humans to do the work that actually requires humanity.

Not matching as a marketplace. Matching as a high-context decision system.

Not a company trying to sound futuristic. A company trying to make family-building less chaotic for the people brave enough to enter it.

I do not want Patriot Conceptions to adopt AI late because the industry finally decides it is safe to talk about.

I also do not want us to adopt AI recklessly because the technology is exciting.

The position I believe in is harder: lead early, build carefully, and let the operating reality teach us what is actually useful.

That is why the ChatGPT Pro gift belongs at the beginning of this issue. It is a symbol of early adoption, but it is also a reminder that tools are only tools. The frontier is not valuable because it is new. It is valuable when it helps us become more accountable to the people we serve.

For Patriot Conceptions, AI is not a replacement for trust.

That is the company we are building: privacy-first, human-owned, AI-native, and serious enough to bring frontier technology into one of the most personal journeys a family can take.

AI Is How We Keep the Promise

Patriot Conceptions received its second Vet100 recognition in March 2026 at Veterans EDGE in Dallas. I am grateful for it, but I do not want to treat it as a trophy story.

The more honest version is this: the award belongs to an ecosystem.

It belongs to the people who created rooms where veteran entrepreneurs can be taken seriously before they are famous. It belongs to the institutions that decided veteran-owned companies should not be an afterthought in the American economy. It belongs to the military spouses, mentors, investors, founders, operators, and quiet encouragers who have carried this work long before Patriot Conceptions had any meaningful public signal.

That is why Dallas felt less like a finish line and more like a reminder of obligation. Vet100 is meaningful because it sits inside a larger architecture of trust: Syracuse University's D'Aniello Institute for Veterans and Military Families, JPMorgan Chase, Inc., Fiserv, and many others who have chosen to build real pathways for veterans moving from service into entrepreneurship.

The military teaches people to respect integrators.

An integrator is not always the person standing at the center of the photograph. Often, the integrator is the one who makes the mission possible by connecting the people, capital, standards, logistics, language, and timing that otherwise stay fragmented.

Syracuse University's IVMF has become one of the rare institutions that can convene across sectors without flattening the veteran experience into slogans. JPMorgan Chase has helped bring serious capital, corporate discipline, and national scale to veteran entrepreneurship. Inc. has given the Vet100 platform a business language that founders, customers, lenders, and media can understand. Fiserv's presence matters because financial infrastructure is not decorative for entrepreneurs. It is part of whether a company can actually operate, receive payments, manage growth, and earn trust.

Each of those organizations does something different. Together, they form a system.

That is what veterans need after service. Not one more isolated program. Not one more inspirational panel that dissolves the next morning. We need systems that hold: education, mentorship, capital, markets, benefits navigation, family support, and a network strong enough to survive the messy middle of building something.

Patriot Conceptions is a small part of that larger story. I am proud of the recognition, but I am more grateful for the standard the ecosystem sets. It asks a founder to grow, but also to become more useful.

One of the strongest reflections from Dallas came from learning more about Dan D'Aniello's story.

He is known in business as a co-founder of The Carlyle Group. He is known at Syracuse as an alumnus and Navy veteran whose philanthropy helped shape the D'Aniello Institute for Veterans and Military Families. But what stayed with me was not the scale of the gift alone. It was the posture behind it.

Great philanthropy is not only generosity. It is judgment about where institutions are needed.

Dan D'Aniello did not simply support a program. He helped strengthen a platform that could serve veterans and military families across transitions, careers, companies, and decades. That is a different category of giving. It says the community deserves durable capacity, not episodic charity.

That point stayed with me in a very practical way.

One of the photos from the evening is not a polished handshake photo. It is a table, a program, a blue-lit ballroom, and the Vet100 mark in the background. I like it because it captures the afterthoughts better than any posed image could. After the awards, after the speeches, after the applause, the real question is quieter: What should we now build with the privilege of being in this room?

My answer, at least for Patriot Conceptions, keeps coming back to military and veteran family building.

Military and veteran families often live inside systems that were not designed around fertility timelines.

Orders move people. Deployments interrupt calendars. PCS cycles change clinics, insurance networks, attorneys, and support systems. Guard and Reserve families can move between civilian and military coverage realities. Veterans may begin the family-building process later because service delayed the choice, complicated the path, or changed the body. Spouses often carry the hidden operational burden: appointments, records, medication schedules, employment disruption, travel, childcare, and the emotional labor of making a complicated system feel survivable.

Fertility care is already hard for civilian families. Military life adds a second operating system on top of it.

Surrogacy adds still another layer. It is clinical, legal, financial, emotional, and deeply personal at the same time. The intended parents need clarity. The surrogate needs protection and respect. The attorneys, escrow partners, insurance reviewers, clinics, agencies, and coordinators all have to move with precision. A mistake in one lane can create fear in every other lane.

This is why I am motivated to build Patriot Conceptions differently.

I do not want this work to be only an agency transaction. Transactions matter, but they are not enough. I want us to build an operating model that respects the full reality of military and veteran families: benefits navigation, clear process design, ethical matching, transparent communication, culturally competent care, financial literacy, legal coordination, and a community where people are not made to feel alone because their family-building path is complex.

The veteran community understands something about trust that this industry needs.

Trust is not a mood. It is a standard. It is built through accountability, follow-through, clarity under stress, and the willingness to put the person next to you before the easiest short-term outcome. In surrogacy, that standard matters because the stakes are intimate. A journey cannot be humane if people feel managed instead of accompanied.

There is a danger in awards. They can make a founder think the story is about the founder.

Vet100 reminded me that Patriot Conceptions exists because other people built first. Syracuse IVMF built first. JPMorgan Chase built first. Inc. built a platform that translates veteran-owned growth into a public business signal. Fiserv and other partners built infrastructure around the entrepreneurs in the room. Dan D'Aniello helped build institutional capacity at a scale that will outlast any one conference or company.

That is the kind of building I respect most.

So the question for us is not whether Patriot Conceptions can be recognized again. The question is whether we can become the kind of company that deserves to be integrated into something larger than itself.

We need clinics that understand military timing. We need attorneys who can make surrogacy navigation clear across state lines. We need fertility benefit experts who can explain what is covered, what is not, and what alternatives exist without making families feel ashamed for asking. We need employers, philanthropists, veteran-service organizations, spouse networks, and military-affiliated founders willing to collaborate without turning every problem into a turf line.

The military and veteran fertility conversation should not be designed only by the loudest people in the room. It should include intended parents, surrogates, military spouses, wounded veterans, LGBTQ+ service members and veterans, clinicians, lawyers, policymakers, insurers, chaplains, commanders, and families who tried to navigate the system and found out where it failed.

Patriot Conceptions can help build that table. We cannot be the table alone.

Not humbled in the performative sense. Humbled because the room made the assignment bigger.

The second Vet100 recognition tells me that Patriot Conceptions is gaining traction. The ecosystem around Veterans EDGE tells me that traction should be converted into service. Dan D'Aniello's example tells me that serious builders think institutionally. The military and veteran families we serve remind me that the work is personal long before it is strategic.

That is the bridge I want to keep walking.

We are building in fertility and surrogacy because family formation is part of readiness, recovery, dignity, and belonging. We are building for military and veteran families because the people who serve should not have to translate every part of their life to systems that were not designed for them. We are building with partners because no single company can solve a field-level problem alone.

The Award Belongs to the Ecosystem

Last week, May 6-8, 2026, I had the privilege of being at the Hoover Institution at Stanford as a Bochnowski Family Veteran Fellow. I arrived thinking the week would be about leadership. It was. But by the end, the deeper lesson was fellowship.

There are rooms where history feels abstract, and there are rooms where it becomes human. Spending an evening learning from Robert Gates, Condoleezza Rice, Jim Mattis, H.R. McMaster, C.Q. Brown, and Admirals Roughead and Ellis was one of those moments that compresses a lifetime of public service into something very immediate. Their careers sit inside wars, crises, alliances, institutions, and decisions most of us only study from a distance. To sit there as an Army veteran and absorb even a small part of that judgment was profoundly humbling.

The humility matters. The point was not celebrity proximity. It was the opposite: a reminder that serious leadership is usually quiet, disciplined, and accountable to people who will never see the deliberation behind the decision.

What stayed with me was not a single line or one polished leadership formula. It was the pattern. Leaders who have carried national responsibility spoke with an awareness of consequence. They had confidence, but not casualness. They respected complexity without hiding behind it. They understood that institutions are made out of people, but also that people need institutions when the pressure gets real.

That is a hard balance. The military teaches pieces of it early: standards, chain of command, mission clarity, after-action learning, and the obligation to the person on your left and right. But the Hoover setting expanded the frame. It asked us to think about service across a longer arc: how democracies preserve trust, how leaders make decisions with incomplete information, how teams hold together when the facts are moving, and how memory becomes a civic resource rather than just nostalgia.

For me, that is the Hoover tone at its best: serious, historically grounded, and unsentimental about responsibility.

The rest of the program moved from statesmanship into practice. We learned from the Stanford d.school, the Stanford Storytelling Project, and others who pushed us to sharpen how we listen, frame, communicate, and make ideas usable.

That might sound softer than national security leadership, but it is not. Design thinking and storytelling are not decorations when the work involves people under strain. They are tools for making reality legible. A good story can carry memory across a group. A good design process can surface the problem nobody wants to name. A good prototype can make a mission concrete enough that other people can join it.

That was the through-line for me. Leadership is not only the moment of command. It is the discipline of making meaning, building trust, and giving people a shared way to move.

The strongest part of the week was the fellowship itself.

Veterans do not need to have served in the same branch, rank, unit, era, or conflict to recognize something in each other. There is a speed of trust that comes from shared obligation. There is also a kind of unconditional regard that is difficult to explain without making it sound sentimental. It is not soft. It is forged out of the knowledge that people can be very different and still be bound by a covenant of care.

That camaraderie was the emotional center of the program. The lectures mattered. The workshops mattered. The access mattered. But the fellowship was the thing that turned a program into a living institution.

I kept thinking about Patriot Conceptions and family building.

Surrogacy is often described through contracts, clinics, screenings, milestones, payments, and legal steps. Those are essential. They protect everyone. But they are not enough to carry the human reality of the journey. The intended parent experience depends on trust. The surrogate experience depends on trust. The relationship between them cannot be reduced to process, even though process must be excellent.

At its best, surrogacy asks for a rare kind of disciplined love: love with boundaries, trust with accountability, generosity with clarity, and hope carried by people who are not all experiencing the journey from the same position. That is why I resist describing family building as a transaction. The transaction has to be right, but the relationship is what makes the journey humane.

The fellowship reminded me that trust is not a feeling we can assume. It is an operating standard we have to earn.

First, build systems that make trust easier to keep. A family-building journey has too many handoffs to rely on charisma or memory. If a promise is made, the system should help the team remember it. If a next step matters, the participants should not have to wonder who owns it. If uncertainty is unavoidable, communication should still be clear.

Second, tell the story with enough precision that people can act on it. The Stanford Storytelling Project reinforced something I feel every day in healthcare-adjacent work: vague inspiration is not enough. People need language that helps them decide, coordinate, and endure. A story should reduce confusion, not inflate the speaker.

Third, protect the fellowship. In any serious mission, the work is sustained by people who trust one another before the outcome is certain. Veteran communities know this. Strong families know this. Great teams know this. Surrogates and intended parents, at their best, are trying to build something in that same moral neighborhood: a relationship capable of carrying fear, waiting, disappointment, joy, and responsibility without collapsing into suspicion.

That is the standard I want to keep learning from.

I went to Hoover grateful for the chance to sit close to history. I left more aware of the obligation to be worthy of the people beside me. For an Army veteran, that is familiar language. For an entrepreneur in family building, it is also a strategy.

The fellowship is not outside the work. The fellowship is the work.

The Fellowship Is the Work

I went to the 2026 Orange County Regional Economic Outlook expecting a regional macro conversation: housing, labor, AI, inflation, supply shocks, and the usual question of whether Orange County can keep growing. I left thinking about intended parents.

That may sound like a strange bridge, but it is the bridge I kept seeing in the data. Family building is deeply personal. ART is medical. Surrogacy is legal, ethical, and relational. But none of it happens outside the economy. The timing of an egg retrieval, the decision to preserve fertility, the choice to use donor eggs, the ability to keep insurance coverage, the cash reserve for a transfer cycle, the escrow plan for surrogacy, and the willingness to begin a multi-year path all sit inside the same household balance sheet.

I am a UCLA Anderson alumnus and serve as an advisory board member at the Price Center for Entrepreneurship. That is one reason I try to stay close to economic rooms, not just healthcare rooms. Fertility care is not only a clinical service. It is a planning system for families making irreversible decisions under uncertainty.

IVF will keep becoming more mainstream as a share of family formation, but access will stay uneven. Surrogacy will not become a volume story; it will become a trust, legal certainty, and planning story.

The Orange County forecast does not prove that IVF demand will rise. It does not predict surrogacy volume. It does not tell any family what to do. What it does is make the planning environment visible: high fixed costs, weak job creation, aging demographics, falling birth counts, expanding healthcare employment, and a benefits landscape that still leaves many families exposed.

1. Orange County Is Prosperous, But The Family-Formation Math Is Tight

The local Forecast materials described a county with output and income strength but very limited job creation. Orange County lost about 1,600 jobs in 2025, while healthcare added roughly 11,000 jobs and carried most of the positive job growth. The same materials pointed to a stubborn housing market: high prices, low affordability, and a regional pattern of families leaving for more affordable places.

The demographic section is where the ART relevance becomes clearest. The forecast shows births falling from 29,977 in 2025 to 25,352 in 2030, while deaths rise from 23,063 to 25,883. By 2030, the natural increase turns slightly negative. K-12 enrollment is also forecast to keep falling, after a long decline tied to fewer births and family outmigration.

That is not an IVF chart. It is a family-formation chart. It says the region is struggling to keep families in place and to replace its population naturally. In a high-cost county, the decision to have a child is increasingly entangled with housing, commute patterns, job security, parental age, and liquidity.

2. IVF Has Crossed A Mainstream Threshold

Nationally, the ART picture is moving in the opposite direction. 

 that 2024 data showed 449,772 IVF treatment cycles, 97,549 cycles with a live birth, and 100,158 babies born through IVF in the United States. That is the first year above 100,000 IVF babies in a single year.

This is happening while total U.S. fertility remains weak. CDC/NCHS final 2024 data show 

, up 1% from 2023, but the general fertility rate declined to 53.8 births per 1,000 females ages 15-44. In plain English: more babies were born in 2024 than in 2023, but the rate at which women of reproductive age had births still fell.

That combination matters. ART can grow as a share of family formation even when the overall birth market is flat or shrinking. The market is not just people who already use IVF. It includes people delaying parenthood, people preserving fertility, LGBTQ+ families, single parents by choice, cancer survivors, people using donor eggs or donor sperm, and intended parents who need a gestational carrier.

3. The Access Problem Is Still The Business Problem

KFF found in its 2024 Women’s Health Survey that 

 said they or a partner had ever needed fertility services, 10% had received services, and 3% needed care but could not obtain it. Cost was the leading reason. KFF also reported that only about 27% of large firms offering health benefits covered IVF services in 2024.

This is why I think employer benefits will remain one of the most important channels in fertility care. Families do not only ask, “Can I medically do IVF?” They ask:

Will my plan cover diagnostics, medications, retrieval, transfer, and storage?

Does coverage apply to donor gametes, fertility preservation, or gestational carrier-related services?

How much cash do we need if authorization, reimbursement, or timing breaks?

In a low-fire, low-hire labor market, these are not administrative details. They are part of the emotional risk families carry before they ever reach the clinic.

4. California Is A Case Study In The Next Coverage Fight

California’s SB 729 is a major 2026 signal. The law requires large-group health care service plan contracts issued, amended, or renewed after implementation to cover infertility diagnosis and treatment, including up to three completed oocyte retrievals and unlimited embryo transfers when medically appropriate. It also prohibits denial based on participation in services involving a donor, gestational carrier, or surrogate. The California budget process later 

delayed implementation to January 1, 2026

The important lesson is not just California. It is the gap between public support for IVF and paid access to IVF. State mandates help, but they do not solve every plan type, every employer structure, every service category, or every family pathway. The next coverage fight will be less about whether people say they support IVF and more about who pays, what is included, and whether third-party reproduction is treated as real family-building care.

5. Surrogacy Will Be Judged By Trust, Not Volume

ASRM’s policy framing is useful here: gestational carrier care remains a specialized modality within ART, governed by state law, FDA requirements, and clinical guidance. ASRM cites GC pregnancies as a small share of deliveries and emphasizes safeguards such as informed consent, independent legal counsel, medical decision-making authority for the carrier, and single embryo transfer when recommended. 

Surrogacy is not going to be evaluated like a consumer marketplace where more volume automatically means more success. It will be evaluated by whether families, carriers, agencies, clinics, lawyers, escrow providers, and courts can trust the process. Parentage clarity, funds flow, insurance continuity, clinical standards, psychological screening, and contingency planning will matter more, not less, as the public conversation around embryos and assisted reproduction becomes more politicized.

For ART, the question is not just medical eligibility. It is whether the plan can survive timing, benefits, liquidity, legal, and emotional uncertainty.

The practical planning lens I would use with intended parents: medical timeline, legal and escrow timeline, employment and benefits timeline, and household liquidity timeline.

This is where I think fertility organizations can do better. Too much of the journey is still explained in silos. Clinics explain clinical steps. Lawyers explain contracts. Agencies explain matching. Benefit managers explain coverage. Escrow providers explain payments. Families experience all of it at once.

The best operators in ART and surrogacy will not only be the ones with good individual workflows. They will be the ones that help families understand the entire plan before the expensive and emotional parts begin.

IVF keeps gaining share of family formation, especially as parenthood shifts later and egg freezing, donor eggs, embryo banking, and frozen transfer workflows become more normal.

Access remains stratified. Employer benefits and state mandates expand the front door, but cash-pay families, small employers, self-funded plans, Medicaid populations, and nontraditional family pathways still face uneven coverage.
